 Permission form needed for Case studies (Health) for a book
Hi all,
Looks like a great forum here.
I am writing a book (health and sleep) based on my experience with my
clients/patients. I would like to use ‘real’ case studies. Does
anyone know if there is a standard form that I can use to obtain
permission from my clients to use their stories and get agreement on
the way I disguise details to protect privacy.
If no such standard permission form exists, could anyone direct me to a lawyer who has experience in formulating one. Maybe there is a rule book somewhere about blending cases for the same effect.
